What government agency do you contact when you have an adverse reaction?

I have, in the past, compiled a few reference files for use by fellow sufferers. Most are found at the IOSV site. This one is not published, and I thought some members here might find it useful. It is as current as I have been able to make it (to date). REMOVE THE ASTERISKS BEFORE YOU LINK.
